For new appointment of AO and POC, a covering letter from respective Ministry / Department must be submitted together with the completed application forms

For users, completed forms must be verified and authorized by the registered Ministry AO and POC only.

All forms must be submitted together with photocopy of IC that has been signed by the applicant.
